About Us

Our Services

Twins HomeCare is proud to offer home and community-based services in accordance with Minnesota’s 245D waiver programs, serving individuals in the Twin Cities area. Our services focus on empowering individuals to live independently in their homes and participate fully in the community. We specialize in providing Individualized Home Supports (IHS), offering support and training to adults aged 18 and above in their own living units or in the community.

Our support services include cueing, skill maintenance, guidance, instruction, assistance with activities of daily living, coordination of community living activities, and direct supervision. Our training services are tailored to each person’s specific needs and encompass skill-building and instructional services designed to acquire, retain, and improve the individual’s experience living in the community. We are dedicated to helping each person achieve greater independence and a higher quality of life through personalized support and training.

1. 24-hour emergency assistance

We provide dependable support around the clock to help individuals feel safe and cared for during unexpected situations. Our team offers timely assistance, reassurance, and support based on each person’s needs. Whether an individual requires immediate help or additional supervision during a challenging moment, we work to promote safety, comfort, and peace of mind for individuals and their families.

2. Adult Companion Services

Our adult companion services focus on building meaningful relationships and reducing feelings of loneliness and isolation. Companions spend quality time with individuals through conversation, games, hobbies, reading, and other enjoyable activities. We also encourage social engagement and participation in everyday routines, helping individuals maintain emotional well-being, confidence, and a greater sense of connection.

3. Night Supervision

Our night supervision services provide attentive overnight support for individuals who may need additional monitoring, reassurance, or assistance during the night. Depending on individual needs, support may include checking on comfort and safety, helping with nighttime routines, and responding to assistance requests. This service helps promote a more secure and restful environment for individuals and reassurance for their families.

4. Respite Care (In-Home or Out-of-Home)

Caring for a loved one can be rewarding, but family caregivers also need time to rest and manage their own responsibilities. Our respite care services provide temporary support while a regular caregiver takes a break. Care can be arranged in the individual’s home or in an appropriate out-of-home setting, depending on availability and assessed needs. We aim to maintain familiar routines, comfort, and continuity of care while giving families valuable time to recharge.

5. Individual Community Living Support

We help individuals participate more confidently in everyday community life while developing independence and social connections. Support may include attending appointments, visiting community spaces, accessing local activities, practicing daily living skills, and building communication and decision-making abilities. Services are tailored to each person’s goals and preferences, encouraging meaningful participation and greater confidence in navigating their community.

6. Individualized Home Supports

Our individualized home supports are designed around each person’s abilities, preferences, and daily living goals. Assistance may include developing routines, organizing personal tasks, maintaining a comfortable home, practicing independent living skills, and participating in everyday activities. We focus on promoting choice, self-confidence, and independence while providing the level of support needed for individuals to live as successfully as possible in their own homes.

7. In-Home Support Services

(a) Individualized Home Supports With Training

This service helps individuals develop the practical skills and confidence needed to manage daily life more independently. A support worker provides guidance and hands-on training based on the individual’s goals, which may include meal preparation, household organization, personal routines, planning, and other daily living activities. Training is adapted to each person’s learning style and abilities, encouraging progress at a comfortable pace.

7. In-Home Support Services

(b) Individualized Home Supports Without Training

We provide practical assistance with everyday activities for individuals who need support at home without a primary focus on skills training. Depending on the person’s assessed needs, this may include help with household routines, meal preparation, organization, and other daily living tasks. Our approach emphasizes comfort, dignity, personal preferences, and maintaining the individual’s quality of life in a familiar environment.

7. In-Home Support Services

(a) Individualized Home Supports With Training

Our family training services help family members and caregivers build the knowledge and practical skills needed to support their loved one effectively at home. Training and guidance are tailored to the individual’s needs and may focus on daily routines, communication, promoting independence, and consistent support strategies. By working collaboratively with families, we aim to strengthen confidence, encourage continuity of care, and create a supportive home environment.

8. Homemaker Services

We help individuals maintain a clean, comfortable, and organized living environment through practical household assistance. Homemaker services may include light housekeeping, laundry, meal preparation, organizing household essentials, and other routine domestic tasks. By helping manage everyday responsibilities, we support individuals in maintaining their independence and enjoying a safer, more comfortable home.
